• Your email address, if you choose to provide it.
• A short message about how we can help, if you choose to provide one.
• The doctor, hospital, treatment, specialty, service or article context from which you started the enquiry, when applicable.

Context and attribution

The site may store the page where you started, basic campaign parameters, enquiry timestamp and a submission reference. Campaign and navigation details may be kept in session storage. Contact details and messages stay in the form until you submit them. Conversion events contain event names only.

How information is used

Enquiry information is used so DM Flymeds can review the request, coordinate a response and manage the enquiry. The form is not intended for detailed medical records, diagnosis or emergency care.

What not to submit

Please do not submit detailed medical histories, prescriptions, test reports, government identification numbers or payment information through this form.

Retention and privacy requests
